Job summary

We are looking for an enthusiastic, caring and motivated Practice Nurse to join our friendly and supportive team at Stanmore House Surgery. 20-25 hour per week.

This is an exciting opportunity for an experienced Practice Nurse, or a nurse looking to develop their career within General Practice, to become part of a well-established team committed to providing high-quality, patient-centered care.

The successful candidate will work as part of our multidisciplinary clinical team, delivering a range of nursing services which may include long-term condition management, cervical screening, childhood and adult immunisations, wound care, health promotion and preventative care, depending on experience and competencies.

Main duties of the job

The ideal candidate should have general practice nurse skills and be competent in chronic disease management with evidence of accreditation. The skills would also include:

  • Cytology
  • Child Immunisations
  • Removal of Sutures
  • Travel vaccinations
  • Family Planning
  • Wound Care
  • Diabetes
  • INR (management of anticoagulation)
  • Injections and Vaccinations as per the national immunisation programme.
  • General Nursing duties as required.
  • Telephone and face to face consultations are a key part of the job, so patient care and good communication skills are essential.
  • Provide comprehensive nursing care to patients within the GP surgery setting
  • Provide health education and counselling to patients and their families

Requirements:

Registered Nurse qualification and current registration with the Nursing and Midwifery Council (NMC).

Previous experience working in a primary care or GP surgery setting is essential.

Sound knowledge of general practice nursing procedures and best practices.

Competency in conducting patient assessments and managing common health conditions.

Proficient in administering medications and treatments.

Strong communication and interpersonal skills, with the ability to build rapport with patients of all ages.

Excellent organisational skills and ability to prioritise tasks effectively.

Familiarity with electronic medical records (EMIS) systems is preferred.

Job description

The Practice Nurse will work as part of the multidisciplinary team to provide high-quality, patient-centered nursing care to the practice population.

The role will include long-term condition management, cervical screening, adult and childhood immunisations, wound care, health promotion, preventative care and other clinical procedures, according to the post holder's experience, training and competencies.

The successful candidate will:

  • Deliver safe, effective and compassionate nursing care within their scope of practice.
  • Support the management and monitoring of patients with long-term conditions.
  • Undertake cervical screening, immunisations, wound care and other treatment-room duties where appropriately trained.
  • Provide health promotion, lifestyle and preventative healthcare advice.
  • Maintain accurate and contemporaneous electronic patient records.
  • Contribute to call and recall systems, clinical audits and quality improvement.
  • Maintain high standards of infection prevention and control, safeguarding, confidentiality and information governance.
  • Maintain current NMC registration, professional development and revalidation requirements.
  • Work collaboratively with GPs, nurses, Healthcare Assistants, administrative colleagues and the wider multidisciplinary team.
  • Participate in relevant practice meetings, training and service development.
