We are a friendly and supportive training practice in Hampshire and are looking for a Practice Nurse to join our team.
Experience and skills in chronic disease management would be desirable, but are not essential, as we are happy to support the right candidate to develop their skills and experience within the role.
Start date: As soon as possible
Hours: Part-time / full-time (up to 37.5 hrs) per week
Working pattern: Monday to Friday – flexible working pattern available plus ad hoc extended hours
Salary: Negotiable depending primary care experience & clinical qualifications
This is an exciting opportunity to join a fantastic, forward-thinking and diverse primary care team at Watership Down Health.
Due to our expanding patient population and increasing demand across primary care, we are looking for an experienced Practice Nurse with a strong background in long-term conditions, to join our friendly and supportive clinical team.
The role is available part time or up to full-time hours per week and will involve working across our three sites:
- Overton Surgery
- Kingsclere Health Centre
- Oakley Surgery
A full driving licence and access to a car are essential, together with the flexibility to work across our three sites and support the needs of our busy practices.
About the Role
The successful candidate will ideally have previous experience working within Primary Care and will be confident managing patients with long-term conditions.
The role will include:
- Chronic disease management, particularly Asthma, COPD and Diabetes
- Supporting the practice in achieving QOF and other quality targets
- Childhood and adult immunisations
- Cervical cytology
- Wound care and compression therapy
- Infection prevention and control
- Associated clinical administration and patient follow-up
- Working collaboratively with the wider multidisciplinary team
- Providing safe, effective and patient-centred care
Essential Requirements
- Current NMC registration
- Previous experience working as a Practice Nurse in Primary Care
- Experience in chronic disease management
- Good experience in respiratory disease management
- Competence in childhood and adult immunisations
- Competence in cervical cytology
- Experience in wound care and compression therapy
- Good communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Flexible, motivated and committed to providing high-quality patient care
This role will require an Enhanced DBS Clearance.
Full driving licence and access to a car.
Flexibility to work across our three practice sites.
Desirable
- Experience of insulin initiation and diabetes management
- Experience in infection prevention and control
- Experience of working within a GP training practice
