Job summary
Are you an Advanced Clinical Practitioner? Primary Care 24 (PC24) are seeking highly skilled and motivated ACPs & ANPs to join our workforce with opportunities within Primary Care. We currently have a salaried part time (22.5 hours role available)
Main duties of the job
Undertake comprehensive health assessments for adults and paediatric patients, applying advanced clinical judgement and holistic care planning.
Assess, diagnose, plan, implement and evaluate treatment and interventions for undifferentiated and undiagnosed conditions.
Perform clinical examinations and interpret findings from both physiological and psychological perspectives.
Manage a clinical caseload with a high degree of autonomy, making safe and timely decisions within agreed governance frameworks.
Contribute to a culture of patient safety, quality improvement and effective multidisciplinary working.
Maintain professional competence and development in line with regulatory requirements, contributing positively as a senior clinician within the practice team.
The Primary Care role is designed for ACPs who feel they are confident working autonomously in a GP practice setting. The post holder will have access to GP clinical supervision and clear escalation pathways, with support focused on safe decision-making, governance and clinical case discussion. PC24 promotes a pragmatic and inclusive working environment, where clinicians are encouraged to seek advice when required and professional accountability is shared within established systems of care.
Job description
Salary: £64,876.00 - £66,711.00 per annum (pro rata)
Location: Various including 15 Sefton road, Litherland & Crosby/Crossways
Shifts: We can package any of the below together to suit your availability -
- Monday 10.30am-6.30pm (15 Sefton Rd)
- Wednesday 8am-4pm (Crosby/Crossways)
- Friday 8am-4pm (Litherland)
