We are looking for an experienced and compassionate Senior Care Practitioner (Nights) to join our dedicated team and help ensure residents receive outstanding care and support throughout the night.

As a Senior Care Practitioner, you will play a key role in leading the night care team, combining hands-on care with leadership responsibilities. You will help create a calm and reassuring environment for residents while ensuring high standards of person-centred care are maintained at all times.

Main duties of the job

We are seeking a motivated and caring Senior Care Practitioner (Nights) to join our team at Parklands Care Home.

The successful candidate will support the delivery of high-quality, person-centred care while providing guidance and leadership to colleagues during night shifts. Working closely with residents, families, healthcare professionals and the wider team, you will contribute to the smooth operation of the home and help ensure residents receive care tailored to their individual needs and preferences.

This role requires a confident and compassionate individual who can support colleagues, make informed decisions and promote a positive culture of care.

Job description

  • Provide leadership, supervision and support to the night care team.
  • Deliver and oversee high-quality, person-centred care in line with residents' individual care plans.
  • Promote residents' dignity, independence, privacy and wellbeing at all times.
  • Monitor residents' health and wellbeing throughout the night and respond appropriately to any concerns.
  • Support the implementation and review of care plans and risk assessments.
  • Maintain accurate and up-to-date records and documentation.
  • Coordinate and delegate tasks effectively to ensure safe and efficient care delivery.
  • Liaise with healthcare professionals, emergency services and relatives when required.
  • Ensure effective communication and continuity of care through detailed shift handovers.
  • Support the induction, mentoring and development of new and existing team members.
  • Promote compliance with safeguarding, health and safety, infection prevention and control, and company policies.
  • Participate in audits, quality assurance activities and continuous improvement initiatives.
  • Contribute to maintaining a calm, safe and well-managed environment throughout the home.
  • Act as a role model for professionalism, teamwork and excellent care practice.
  • Uphold the values and reputation of Parklands Care Home and Sandstone Care Group at all times.
  • Undertake any other duties reasonably required to support the effective operation of the home.
