Main duties of the job

We are seeking a caring and enthusiastic Care Practitioner to join our team at Parklands Care Home.

The successful candidate will support the delivery of high-quality care tailored to each resident's individual needs, preferences and choices. You will work closely with colleagues, residents, families and healthcare professionals to promote wellbeing, independence and quality of life while helping to maintain a safe and supportive environment throughout the home.

Job description

  • Deliver high-quality, person-centred care in accordance with individual care plans.
  • Support residents with daily living activities while encouraging independence and choice.
  • Promote residents' dignity, privacy, comfort and wellbeing at all times.
  • Build positive and professional relationships with residents, relatives and visitors.
  • Encourage participation in social, recreational and meaningful activities.
  • Monitor residents' wellbeing and report concerns or changes appropriately.
  • Support residents with nutrition, hydration and general wellbeing needs where required.
  • Provide companionship, reassurance and emotional support to residents.
  • Maintain accurate records and documentation in accordance with company policies and procedures.
  • Work collaboratively with colleagues to ensure continuity and consistency of care.
  • Contribute to maintaining a safe, clean and welcoming environment throughout the home.
  • Follow safeguarding, health and safety, infection prevention and control procedures at all times.
  • Participate in mandatory training and ongoing learning and development opportunities.
  • Uphold the values and standards of Parklands Care Home and Sandstone Care Group.
  • Support colleagues and contribute positively to a culture of teamwork, respect and professionalism.
  • Undertake any other duties reasonably required to support the effective operation of the home.
