Main duties of the job

We are seeking a caring and enthusiastic Care Practitioner to join our team at St Helens Hall Care Home.

The successful candidate will support residents with their daily living needs, ensuring care is delivered in a way that promotes choice, dignity and respect. Working collaboratively with colleagues, residents, families and healthcare professionals, you will contribute to maintaining high standards of care and enhancing residents' quality of life.

This role is ideal for someone who is passionate about caring for others and committed to delivering exceptional person-centred support.

Job description

Main Duties of the Job

  • Deliver high-quality, person-centred care in accordance with individual care plans.
  • Support residents with daily living activities while promoting dignity, independence and choice.
  • Assist residents to maintain their wellbeing, comfort and quality of life.
  • Encourage and support participation in social, recreational and wellbeing activities.
  • Build positive and professional relationships with residents, relatives and visitors.
  • Monitor residents' wellbeing and report any concerns or changes appropriately.
  • Support residents with nutritional, hydration and general wellbeing needs where required.
  • Provide companionship, reassurance and emotional support to residents.
  • Maintain accurate records and documentation in line with company policies and procedures.
  • Work collaboratively with colleagues to ensure continuity and consistency of care.
  • Contribute to maintaining a safe, clean and welcoming environment throughout the home.
  • Follow safeguarding, health and safety, infection prevention and control policies and procedures at all times.
  • Participate in mandatory training and ongoing professional development activities.
  • Uphold the values and standards of St Helens Hall Care Home and Sandstone Care Group.
  • Support colleagues and contribute positively to a culture of teamwork, respect and professionalism.
  • Undertake any other duties reasonably required to support the effective operation of the home.
