This is a unique opportunity to shape a new role and make a real impact across South Yorkshire.

The South Yorkshire Mental Health, Learning Disabilities and Autism (MHLDA) Provider Collaborative is seeking an experienced professional to join its system-wide programme as an Acquired Brain Injury (ABI) Specialist Case Manager. The role has been created to address gaps in care for adults with ABI and co-existing mental health needs, improving outcomes and reducing out-of-area placements.

The Provider Collaborative brings together NHS organisations, commissioners, local authorities, independent providers and people with lived experience to develop specialist services and support people to receive care closer to home.

Hosted by Rotherham Doncaster and South Humber NHS Foundation Trust (RDaSH), this role will work across organisational boundaries to support the development of integrated, person-centred ABI pathways across South Yorkshire and beyond where placements are out of area.

You will manage a complex caseload, providing expert oversight, coordination and risk management while working with partners across the system. This is a rare opportunity to influence service transformation, shape future pathways and improve care for people with complex needs.

Main duties of the job

The post holder will lead the coordination and management of highly complex ABI cases across South Yorkshire, working collaboratively with people with acquired brain injury, families, health, social care, and partner agencies. You will undertake comprehensive assessments, develop and review care and risk management plans, and support safe, effective, and person-centred care delivery.

You will play a key role in reducing out-of-area placements by supporting repatriation planning and improving local care pathways, while also contributing to reduced length of stay and improved patient flow.

The role requires strong leadership and influencing skills, as you will provide expert advice to multidisciplinary teams, support service development, and drive quality improvement initiatives.

You will analyse complex clinical and service data, contribute to business cases, and help shape future models of care for people with ABI.

Working autonomously across organisational boundaries, you will manage competing priorities, respond to complex and high-risk situations, and ensure care delivery aligns with best practice and relevant legislation.
