This is an exciting opportunity to join South West Yorkshire Partnership Teaching NHS Foundation Trust as an Advanced Clinical Pharmacist supporting mental health services at Cheswold Park Hospital, Doncaster. Cheswold Park Hospital is part of the Trusts Forensic Psychiatry services and is a medium secure hospital. This is a brand new post so if you're looking for a role with real influence, professional freedom and the chance to shape how pharmacy supports mental health care, this post offers exactly that.

You'll work with a high level of independence, managing your own workload and leading projects that directly improve medicines optimisation and patient experience. As a senior member of the local pharmacy team, you'll provide advanced mental health medicines expertise, offer clinical leadership, and oversee the management and supervision of pharmacy colleagues.

Your clinical work will include delivering specialist pharmacy services to inpatients at Cheswold Park Hospital and developing the pharmacy service at the site which has until recently been provided by an independent provider.

As part of the wider South West Yorkshire Partnership NHS Foundation Teaching NHS Trust pharmacy service, you will have access to support, training, & supervision from experienced mental health pharmacy professionals. You will also play a key role in trustwide medicines optimisation initiatives & contribute to delivering our new Medicines Optimisation & Pharmacy Strategy.

Main duties of the job

The role of the post holder will include provision of direct patient care, carrying out patient consultations and medication counselling, complex medication reviews, rationalizing and optimizing medicines for mental health conditions as well as concurrent medicines for those with long term physical health conditions, prescribing where appropriate and qualified.

There will also be a role to support other health care professionals in relation to mental health medicines including responding to queries and education and training. This role will provide clinical supervision to other members of the mental health pharmacy team.

Supporting good prescribing practice and supporting nursing practice in respect of safe and secure handling, questioning and challenging where appropriate

Budget analysis and monitoring

Monitoring of high risk and high-cost medicines at a service level.

JOB SUMMARY

Provision of Clinical Pharmacy service to allocated teams

Provision of high quality patient focused pharmaceutical care.

Provison of professional responsibilities of a pharmacist supervising the dispensing of prescriptions at Fieldhead Pharmacy whilst fulfilling the dispensary commitment.

Participation in the Saturday, Bank Holiday and On-call Rotas

In conjunction with the pharmacy team, develop and implement safe and effective policies and procedures in line with New Ways of Working

To provide management to allocated staff including appraisal , development and clinical supervision

To be involved in the development of new services as allocted by pharmacy management team

Develop and participate in Trust-wide medicines management audits and attend allocated governenace groups to represent pharmacy.

KEY RESULT AREAS:

• To be responsible for the clinical pharmacy and prescription monitoring at allocated clinical teams . Be a specialist medication referral point for these teams.

Performance indicators

• Medicines reconciliation targets for inpatient areas
• Ensure all service users have access to clinical pharmacy services
• Screening for all service users admitted to intensive home based treatment services
• Develop and contribute to patient focused services to allocated teams , including service user groups.
• Provide appropriate and timely verbal and written information on all aspects of medication usage to service users, carers and healthcare professionals within SWYPFT.
• Participate in in-house training for pharmacy staff and healthcare professionals , both attending and delivering training.
• Participate in the training of pre-registration pharmacy graduates and mental health pharmacy diploma students.
• Participate in the development of the medicines management clinical audit programme in particular the POMH audits and ensure roll out to allocated units.
• Participate in research and development as required for the development of the clinical pharmacy services.
• To cover for other senior pharmacists in their absence.
• If required due to sickness or other absence to work in other localities of the organisation.
• To carry out CPD in line with the GPhC requirements.
• To maintain and demonstrate a good clinical pharmacist knowledge.
• To work within the policies and procedures of the South West Yorkshire Partnership NHS Foundation Trust and also to ensure compliance with the pharmacy policies and procedures from other organisations when working in partnership.
• To participate in the Trusts Appraisal system.
• Following qualification as an independent non-medical prescriber, prescribe medication as an independent prescriber within their own sphere of competence and the bounds of their approved prescribing status in accordance with the Trust non-medical prescribing policy.
• Promote patient choice and understanding when communicating assessment outcomes or prescribing medicines, including provision of high quality information both written and verbal to support this.
• Be responsible for ensuring the security and safe handling of FP10 prescription forms and to report any loss or theft as per Trust guidelines
