Main duties of the job
To assist medical personnel in the care of Practice patients to include a full range of treatment room services, preventative care, screening and patient education, childhood immunisations and a knowledge of the range of chronic disease conditions.

- 3 GP Partners, 1 practice nurse, 1 HCA
- EPS (electronic prescribing) practice in South Wales.
- Training practice for Medical students, practice Nurses.

We are a forward thinking, enthusiastic, and well thought of practice. We are located conveniently on the M4 corridor in purpose built premises, providing a high standard of care for patients. We provide the dermatology Service to patients in the County Borough area under a Service Level Agreement with the Health Board.
Job description
Coalbrook Surgery is seeking an experienced, enthusiastic and compassionate Practice Nurse to join our friendly and dedicated primary care team.
The successful candidate will work collaboratively with GPs, healthcare professionals and support staff to provide high-quality, patient-centred nursing care to our practice population. The role involves delivering a broad range of clinical services, supporting long-term condition management, promoting health and wellbeing, and contributing to the continued development of nursing services within the Practice.
Main Responsibilities
- Provide assessment, screening, treatment and health education services to patients in accordance with Practice policies, national guidelines and agreed clinical protocols.
- Deliver nursing care independently and collaboratively with GPs and the wider multidisciplinary team.
- Undertake general and specific health assessments and refer appropriately where required.
- Provide health promotion advice and support patients with preventative care and lifestyle changes.
- Conduct chronic disease management reviews, ensuring patients with long-term conditions receive appropriate monitoring and annual reviews.
- Support enhanced services provided by the Practice, including non-complex wound care, administration of Gonadorelins and other commissioned services.
- Undertake clinical procedures including venepuncture, ECGs, wound care, health checks and other investigations within competency.
- Collect, process and dispatch pathological specimens in accordance with Practice procedures.
- Maintain accurate patient records and ensure appropriate documentation of all consultations and treatments.
- Review and action laboratory results within agreed protocols.
- Support Healthcare Support Workers and contribute to the effective organisation of nursing services.
- Participate in clinical audits, quality improvement activities, Practice meetings and governance processes.
- Maintain professional development through relevant training, courses and study days.
- Promote confidentiality, equality, diversity, patient dignity and safe clinical practice at all times.
Essential Requirements
- Registered Nurse with current NMC registration.
- Experience working in primary care or a similar clinical environment.
- Strong clinical assessment and decision-making skills.
- Excellent communication and teamwork abilities.
- Commitment to providing safe, compassionate and effective patient care.
Desirable Experience
- Chronic disease management (diabetes, COPD, asthma, hypertension).
- Cervical screening.
- Immunisation and vaccination programmes.
- Venepuncture and ECG experience.
- Wound care.
- Supporting healthcare assistants and junior staff.

About us
Coalbrook Surgery is in the village of Pontyberem, equidistant from the towns of Carmarthen and Llanelli and is part of the Amman Gwendraeth Cluster. It is a well established, friendly Practice, currently consisting of 3 GP Partners who are supported by an excellent nursing team and an enthusiastic, competent administrative team.
