Job summary

As a phlebotomist, you will be responsible for delivering high-quality care to patients within their own homes, ensuring care is delivered safely, compassionately, and in line with individual needs. You will be responsible for supporting registered nurses by delivering delegated nursing interventions, managing your workload within an agreed scope of practice, and maintaining accurate standards of patient care. You will be responsible for working independently while following clinical guidance, supporting the continuity of community nursing services during periods of increased demand, and contributing to a responsive and resilient service.

You will also have the opportunity to support a range of clinical activities, including enhanced skills such as phlebotomy where appropriate, while working collaboratively across community nursing teams to ensure patients receive consistent, high-quality care.

Responsibilities of this job role

To monitor and accurately document changes in patients' physical or emotional wellbeing, promptly escalating concerns, deterioration, or improvements to the registered nurse while maintaining clear, confidential, and professional records in line with organisational standards.

To carry out a range of delegated clinical procedures within your level of competence, ensuring all interventions are delivered safely, effectively, and in accordance with clinical policies, professional guidance, and individual patient requirements.

To promote a safe care environment by identifying and responding to potential clinical, environmental, or safeguarding risks, following infection prevention, manual handling, lone working, and health and safety procedures at all times.

To communicate effectively with patients, families, carers, and the wider multidisciplinary team, ensuring information is shared appropriately to support coordinated care, encourage patient involvement, and promote independence wherever possible.

To contribute to the continuous improvement of community nursing services by maintaining accurate clinical documentation, complying with governance requirements, participating in audits and quality improvement initiatives, and adhering to all relevant policies and procedures.

To maintain and develop your professional knowledge and clinical skills by participating in mandatory training, clinical supervision, reflective practice, and ongoing learning, ensuring you work within your scope of competence while delivering high standards of community nursing care.
