Job summary
Hawthorn Medical Centre is seeking an experienced and motivated Nurse Practitioner to join our well-established multidisciplinary primary care team.
The successful candidate will play a key role in delivering high-quality, patient-centred care across both acute and chronic disease management.
They will be responsible for assessing, diagnosing, treating, and managing patients presenting with minor illness, whilst also delivering specialist respiratory care services for patients with asthma and COPD.
Working collaboratively within our modern Total Triage Team , the post holder will contribute to improving patient access, supporting continuity of care, and ensuring the delivery of safe, effective, evidence-based clinical services.
Main duties of the job
Minor Illness Management
- Independently assess, diagnose, treat, and manage patients presenting with undifferentiated and minor illness.
- Undertake comprehensive history taking and clinical examinations.
- Request and interpret diagnostic investigations as appropriate.
- Formulate differential diagnoses and management plans.
- Prescribe medication within scope of practice and relevant prescribing qualification.
- Provide health promotion and preventative care advice.
- Identify safeguarding concerns and escalate appropriately.
- Refer patients to secondary care, community services, or other healthcare professionals where required.
Respiratory Role
- Deliver respiratory services for patients with asthma and COPD.
- Conduct annual respiratory reviews in accordance with national and local guidelines.
- Perform and interpret spirometry where qualified and competent.
- Develop personalised care plans for respiratory patients.
- Optimise respiratory medication and inhaler therapy.
- Support achievement of Quality and Outcomes Framework (QOF) respiratory indicators.
- Educate patients regarding self-management and inhaler techniques.
- Work collaboratively with community respiratory teams and secondary care services.

This is an exciting opportunity to work within our developing Total Triage Hub, providing high-quality care for patients presenting with acute minor illness while also supporting respiratory services for patients with asthma and COPD. The successful applicant will work autonomously within their scope of practice whilst benefiting from close collaboration with a highly skilled team of GPs and allied healthcare professionals.
The role offers an excellent balance of acute clinical practice and chronic disease management, making it ideal for an experienced clinician looking to have a meaningful impact on patient outcomes.
You will play a key role in delivering high-quality respiratory services across the practice.
Responsibilities include:
- Managing and reviewing patients with asthma and COPD in line with national guidelines.
- Conducting annual respiratory reviews and developing personalised care plans.
- Performing and interpreting spirometry where appropriately qualified.
- Optimising respiratory medication and inhaler therapy.
- Supporting the achievement of Quality and Outcomes Framework (QOF) targets.
- Educating and empowering patients to self-manage their respiratory conditions.
- Working collaboratively with community respiratory teams and secondary care specialists to deliver seamless patient care.
You will contribute to maintaining the highest standards of patient care through:
- Delivering safe, effective, evidence-based clinical practice.
- Maintaining accurate and contemporaneous clinical records.
- Participating in audit, service evaluation, and quality improvement activities.
- Supporting clinical governance, patient safety, and risk management initiatives.
- Adhering to practice policies, procedures, and infection prevention standards.
