Job summary
Opportunity to work across all our sites engaging with a wide range of different multi-agency professionals to proactively support people with frailty and Long-Term Conditions.
We currently have an exciting opportunity for an enthusiastic and forward-thinking Registered Nurse who is looking to enhance and develop their career in the primary care setting. We need you to help us develop a proactive approach to population health management to identify people who require quality care to manage the impact of their condition. We offer a structured holistic assessment to those identified to be most at risk.
You will be working as an autonomous practitioner as well as collaboratively with the wider integrated frailty service offered by our community colleagues. You will work as part of our Frailty Service within our Wellbeing Team and other practice staff to make this a key role in delivering care to this patient cohort.

Main duties of the job
To assess, plan, implement and evaluate care plans for patients; promoting proactive independence and autonomy; working within a multi-disciplinary team.
To build and continually advance own clinical knowledge, skill and competence based on current evidence through educational programmes.
Provide specialist advice around the management of fragility to others.
To work in collaboration with other professionals within our neighbourhood to deliver and ensure proactive care is provided appropriately to our population
To demonstrate clinical effectiveness of service delivery by use of evidence-based practice and outcome measures.
To have the skills to take a history, assess, examine, diagnose, and develop a management plan.
To recognise, assess and manage risk across the immediate and wider working environment and make appropriate decisions autonomously ensuring statutory requirements are met.
To be responsible for patient safety through knowledge of systems, legal requirements and understanding of litigation.
To communicate effectively verbally and in written form in the exchange of highly complex, sensitive, or contentious information in demanding situations using de-escalation, mediation, resolution, and professional Duty of Candour.
To evaluate care, taking appropriate action leading to improvement in quality standards through clinical audit, root cause analysis and dealing with complaints.

RESPONSIBILITY FOR HUMAN RESOURCES / WORKFORCE
To provide clinical support to and receive clinical support from other colleagues.
To provide regular advice/guidance/support to more junior staff.
To be responsible for teaching and assessing in clinical practice to junior staff.
RESEARCH & DEVELOPMENT
Participate in surveys, regular audits, and clinical trials relevant to the role as required
To take responsibility for keeping abreast of developments and research relevant to specialist clinical work and the profession.
POLICY & SERVICE DEVELOPMENT
Responsible for contributing to the development of policies, procedures, and practices applicable to the role.
RESPONSIBILITY FOR INFORMATION / DATA
To maintain and ensure the highest quality of recording of patient data into the relevant record system,

OTHER RESPONSIBILITIES
To provide proactive, holistic assessments of adults living with frailty working with people and their families/carers to develop and support personalised care planning to promote independence and reduce unplanned admission.
To proactively assess, manage and monitor the health and social care needs of people with complex long term health conditions.
To be able to recognise deterioriating patients and respond appropriately
To work in collaboration with the wider frailty service to promote proactive, enhanced clinical management that reduces unwarrented hospital admissions.
To work flexibly to meet the demands of the service and adapt to changing priorities within the working day/week
ENVIRONMENTAL FACTORS
The role will require developed physical clinical skills where accuracy is important e.g., for patient interventions
About us
We are located in a beautiful part of North Dorset with excellent links both road and rail with London. There are fantastic local schools both state and private. The coast is within easy reach for sailing, diving and outdoor pursuits.
Youll be based across our Blackmore Vale Partnership surgeries (Sturminster Newton, Shaftesbury & Marnhull). Hours are Monday to Friday during the day, and a full driving licence is needed.
