Job summary

As a Caseworker, you will provide comprehensive support to clients experiencing substance use issues, domestic abuse, and homelessness. You will guide clients through their treatment and recovery journey, implementing psychosocial interventions, developing recovery plans, and facilitating referrals to relevant services. Your work will help people achieve recovery.

• Develop and implement tailored recovery plans.
• collaborate with multi-disciplinary teams and external agencies to ensure service users receive the care they need.
• Work proactively and flexibly to engage clients and meet service needs.
• Ensure adherence to all processes and procedures.
• Liaise with the clinical team t co-ordinate appropriate clinical interventions for clients.

Job description

Providing comprehensive care to clients with substance use issues, supporting them from their initial entry into service throughout their treatment and recovery journey.

Completing screenings and assessments, delivering evidence-based interventions, planning for recovery, and completing care reviews.

Identifying appropriate interventions, services, placements, and packages of support.

Presenting funding requests and/or updates concerning client outcomes to an appropriate panel and monitoring/reviewing agreed plans/placements.

Effectively managing a caseload of clients. Regularly reviewing cases, implementing actions, reviewing risks and following all safeguarding processes and procedures.

Developing effective working relationships and communication with key voluntary and statutory services to embed a more solution-focused response to people with multiple needs.

Carry out risk assessments and risk management.

Performing proactive outreach including home visits, appointments in the community and outreach for street active/entrenched groups to enhance engagement and promote social inclusion.

Identifying and addressing barriers/obstacles to treatment and developing innovative solutions to enhance accessibility.

Planning service delivery contact flexibly to meet individual needs and adopting a proactive approach to engagement to maximise service accessibility. This includes delivering interventions peripatetically in a wide range of settings and communication via a range of methods.

Working alongside our specialist nurse prescribers to support our substitution prescribing program.

Working closely with clinical staff to ensure clients have access to appropriate clinical interventions.

Ensuring strict adherence to all clinical pathways, processes and guidelines including titration and medication pathways.

Supporting clients identified through the criminal justice to reduce substance use related harm as well as reducing the risk of reoffending.

Delivering DRR/ATR interventions in alignment with National Probation Service (NPS) court order requirements.

Ensuring seamless transitions between police/court custody suites, prison, ad community settings.

Providing harm reduction information and implementing harm reduction strategies.

Following processes relating to referrals for detox and rehab.

Working alongside any relevant person/services to plan safe exits from service and organising appropriate ongoing care.

Ensuring accurate recording of clients progression, records and plans.

Ensuring clients have regular up-to-date copies of their support plan and safety plan.

Recording activity and results in an accurate and timely manner to support project evaluation as well as funding and research requirements.

Making constructive use of supervision sessions as well as engaging in reflective practice.

Commitment to continuous professional development.

Working appropriately within professional boundaries.
