Job summary

We are seeking an experienced and motivated Physiotherapist to join the Long Covid and Multiple Symptoms Multidisciplinary Service. This role involves assessing and treating patients with complex symptoms such as fatigue, breathlessness, brain fog, pain, anxiety and sleep difficulties across clinic, community, home and virtual settings.

This is a unique therapy-led, medically supported service consisting of occupational therapists, physiotherapist, clinical psychologists, care coordinators and clinical support staffThe service includes two pathways; digital education programme and a Lambeth and Southwark face-to-face assessment, and interventions including 1:1 intervention or a face-to-face group programme. Interventions are provided in a variety of locations including home, clinic, community and virtually.

The post holder will contribute to all aspects of the patient pathway, including assessment, supported self-management and rehabilitation, participate in multidisciplinary discussions, and play an active role in ongoing service development.

This is an excellent opportunity to develop specialist rehabilitation skills within an innovative and expanding service.

Main duties of the job

To perform specialised and advanced assessments and treatments as part of the Post Covid and Multiple Symptoms Service, providing home, outpatient and community-based assessments and intervention to patients across the pathway.
Provide supervision and support development of therapy assistants and students.
To work autonomously, with support as appropriate, frequently as a lone practitioner and at times in conjunction with team colleagues, across a variety of locations within the community and outpatient settings including client's homes and residential care settings to meet the individual needs of the multi-cultural population in an inner-city area.
To work across Lambeth and Southwark, accessing and recording information, communicating with patients and colleagues, attending meetings or training using mobile devices, and coming into offices when required.
Support the service's commitment to meeting client needs and delivering excellence. It is essential the post holder be able to work their hours flexibly and contribute fully to both existing and new rotas. This may include 7 day working and non- traditional shift patterns.

Job description

To be professionally and legally responsible for all aspects of the post-holders own work
To carry out assessment of patients as an autonomous practitioner, including those with complex presentations, and using clinical reasoning skills and knowledge of evidence-based practice, determines appropriate care plan utilising wide range of treatment skills and options
To interpret and analyse clinical and non-clinical facts to form accurate diagnoses in a wide range of conditions including those with a complex presentation, with occasional guidance from specialist physiotherapists
To formulate accurate prognoses and recommend best course of intervention, developing comprehensive discharge plans
To continually reassess patients in order to progress treatments effectively
To co-ordinate intervention which may include other disciplines; advises and educates patient/carers/relatives/other health professionals
Responsible for maintenance of accurate written records using POMR system and use of computerised data collection and records and comprehensive progress and discharge reports to medical referrers as required by rotation
To manage clinical risk within own caseload at all times
To demonstrate physical ability to carry out physiotherapy assessment and interventions including manual handling techniques and therapeutic handling
To demonstrate highly developed dexterity, co-ordination and palpatory sensory skills for assessment and manual treatment of patients

To be able to manage potentially stressful, upsetting or emotional situations in an empathetic manner, including but not limited to those in oncology, ICU, vascular and neurology.

To participate in on call and weekend working at either St Thomas, Guys or Evelina London hospital sites as a lone practitioner
