We're looking for an enthusiastic Clinical Pharmacist to join our well-established and growing pharmacy team at Cruddas Park Surgery and Hillsview Surgery. We are proud to have an experienced pharmacy team comprising two full-time Clinical Pharmacists and a full- time Pharmacy Technician, working closely with GPs, nurses and the wider multidisciplinary team.

Main duties of the job

We are looking for a pharmacist who shares our belief that excellent healthcare means looking beyond the medication list and understanding the whole person sitting in front of you.

You'll have the opportunity to:

  • Undertake meaningful Structured Medication Reviews.
  • Build ongoing relationships with patients.
  • Influence prescribing quality and medicines optimisation.

Job description

The Role

This practice-based role is for 22.5 hours per week.

Your week will be varied and typically split between:

Planned Clinical Sessions

  • Structured Medication Reviews(SMRs).
  • Long-term condition management.
  • Patient-initiated medication reviews.
  • Medicines optimisation.

Duty Pharmacist Sessions

  • Reviewing acute prescription requests.
  • Responding to medication queries from patients and clinicians
  • Supporting safe prescribing across the practice.
  • Working collaboratively with colleagues to resolve medication-related issues.

You'll receive regular support from our Lead Pharmacist and GP Clinical Supervisor, with dedicated pharmacy and practice meetings for case discussion, learning and professional development.

MainDuties

  • Provide expert medicines advice to the multidisciplinary team.
  • Conduct Structured Medication Reviews and medication optimisation reviews.
  • Manage patients with long-term conditions including:

Atrial Fibrillation

Chronic Kidney Disease

Heart Failure

Gout

Hypothyroidism

Hypertension

  • Support nursing colleagues with the management of diabetes and respiratory conditions.
  • Review acute prescription requests.
  • Support medicines reconciliation following hospital admissions and out patient appointments.
  • Help the practice achieve local and national prescribing quality targets.
  • Undertake medicines optimisation and quality improvement projects.
  • Support vulnerable patients and help reduce barriers to accessing health care and medication.
  • Maintain awareness of CQC and QOF medication safety requirements.
