Job summary
The post holder is a Nurse Associate registered with the Nursing and Midwifery Council. They are required to act within their professional boundaries, supporting and working alongside a multi-disciplinary team within general practice. The post holder will contribute to providing nursing care to patients registered in the GP practice. The post holder will focus on assuming a lead role in specified areas, e.g., vocational and employment advice to patients, activities coordination, assisting in individual and therapeutic group work. Work with the qualified nurses to deliver individual treatment packages with an emphasis on collaborative care that focuses on recovery and the need to work towards discharge where appropriate. The post holder will work with other members of the Multidisciplinary Team (MDT) to both organise and facilitate care appropriate to the individual needs of the patient.
Main duties of the job
MANDATORY EXPEREINCE IN CYTOLOGY.
To engage in positive relationships with patients/carers/families and other staff based on respect and within appropriate boundaries.
To act in a professional capacity at all times.
To be able to set therapeutic limits with patients, carers or relatives.
To be able to communicate with the patient about care planning risk assessment needs.
Communicate complex and sensitive information verbally and in writing.
Actively contribute during handovers to meet the care needs of patients.
Actively contribute to staff meetings
To be able to confidently and accurately convene patient information in the role of an associate nurse for MDT meetings.
To be able to articulate an understanding of MDT structures and members roles.
To formulate care plans under the supervision of registered staff.
To communicate risk issues as they arise to ensure safe care.
To communicate concerns relating to patient behaviour or mental state to other members of the multidisciplinary team
Job description
Clinical Duties
- Undertake routine clinical procedures in line with training and competency, including:
- Blood pressure monitoring
- Venepuncture
- ECG recording
- Wound care and dressings
- Injections and immunisations (where trained and competent)
- Health checks and long-term condition monitoring
- Support chronic disease management clinics (e.g. diabetes, asthma, COPD) under supervision
- Recognise and respond appropriately to changes in a patients condition, escalating concerns to a registered clinician
- Maintain accurate and timely patient records using the clinical system (SystmOne)
Patient Care and Communication
- Provide safe, compassionate, and person-centred care
- Communicate effectively with patients, carers, and the wider healthcare team
- Promote health education and self-care to patients
- Maintain patient confidentiality at all times
Team Working
- Work collaboratively with GPs, nurses, healthcare assistants, and the wider primary care team
- Contribute to a positive, supportive team environment
- Participate in clinical meetings and training as required
Governance and Professional Practice
- Work within the Nursing and Midwifery Council (NMC) Code of Conduct
- Maintain registration as a Nursing Associate with the NMC
- Take responsibility for personal professional development and mandatory training
- Adhere to infection prevention and control, safeguarding, and health & safety policies
- Participate in audits and quality improvement activities where appropriate

The practice consists of 1 main site and 3 branch sites, including Mexborough. Staff are expected to work at main and branch sites.

We have under 40 staff including 10 GPs, 2 practice nurses, 3 HCA's, 3 x Practice pharmacist, 2 x Practice pharmacy technician, MSK practitioners.
