Job summary

Come and join our Mountbatten community team, who are experts in end-of-life care provision.

The community service provided by Mountbatten aims to provide assessment, intervention and monitoring of patients with progressive and life limiting conditions enabling them to remain within their preferred place of care.

2 posts available with the option of hours ranging from 22.5 (3 days) to 37.5 (full time) for each post.

Hours to be worked flexibly including weekends, bank holidays, evenings/nights.

A full driving licence and use of vehicle for work purposes with business insurance is essential for this role.

Main duties of the job

The successful candidate will hold a Nursing Associate Foundation Degree, and work as an Associate Practitioner as part of a highly active community team contributing to the provision of 24/7 Specialist Palliative/End of Life Care.

Responsibilities will not only be for providing care, but for providing advice and support to patients, their families and other healthcare professions within the community.

You will work independently to complete full holistic patient assessments to ensure needs are met in a timely manner and liaise with the wider team to manage and escalate any identified needs or concerns.

The post holder will deliver palliative and end-of-life care as part of Mountbattens multidisciplinary community services team providing nursing interventions and ongoing assessment under the direct clinical leadership of the Clinical Nurse Specialists/Senior Nurse on Duty reporting to the Community Team Leader.

Job description

Develop and maintain communications with patients, carers, other professionals and volunteers

Work closely with members of multidisciplinary team and outside agencies to ensure the provision of individualised care

Demonstrate awareness towards of limits of practice and seek support and guidance as appropriate

Escalate any concerns as appropriate

Support with telephone advice for patients, carers and other professionals, as required from the Community Co-Ordination centre.

Give and receive accurate reports and changes in patients condition

Undertake joint assessments with other professionals and liaise as appropriate regarding ongoing care

Be aware of the service health and safety aspects and adhere to lone worker policy

Promote a safe environment for all patients, carers, visitors and staff in the patients preferred place of care.

Providing support for the Inpatient Unit as appropriate.
