Job summary
This post is a 6PA less-than-full-time substantive post for a Specialty Doctor in Elderly care Medicine and Rehabilitation, starting October'26. This post is sought to provide inpatient and community-based care for patients with long term conditions based at Wokingham Community Hospital
Main duties of the job
The Specialty doctor will be expected to attend consultant ward rounds and multidisciplinary team meetings where patient rehabilitation progress is reviewed, and discharge plans are made. The post holder will be directly involved in communication with patients and relatives and able to offer advice to health and social care colleagues regarding rehabilitation on the wards.
Expectation to make every effort that patients are enabled to reach their rehabilitation goals and work with colleagues to ensure that the care to patients is in accordance with the NSF for Older People and other relevant guidance on good practice
