Job summary

We are seeking an experienced and competent nurse with sound clinical skills and strong leadership and management experience to join our Urgent Community Response, Out of Hours service.

You will provide clinical leadership, line management and support to the team, ensuring safe, effective and high-quality patient care.

Please note: Candidates should be aware that approximately 60% of this role will involve working between 20:00 and 08:00, ensuring the team has sufficient clinical leadership and support throughout the Out of Hours period.

Flexibility will be required and candidates should be aware that the hours-of-service delivery are, 8pm-8am, 365 days per year, ensuring the team has sufficient clinical leadership and support throughout the Out of Hours period. It is expected that the role requires the successful candidate to have visibility during daytime working hours to attend (wider MDT discussions and meetings ) to support the operational management of the team. Please note that the OOH team also operates form St James Clinic in Kings Lynn and rotational attendance to provide clinical leadership will be required.

Please note if you are an internal candidate that you would return to your original role after the 12 month secondment has ended.

Main duties of the job

To provide clinical expertise, clinical leadership and line management to staff working within Out of Hours.

To support staff to achieve and ensure competencies are maintained within the services

To contribute to the delivery of high standards of health care to patients within their own places of residence

To ensure that Trust annual priorities and quality goals are embedded within the workforce by ensuring that all annual appraisals are complete within the team.

Job description

To provide professional and clinical leadership demonstrating a supportive and flexible leadership approach and a proactive responsible attitude so as to promote confidence in the team and Service.

To clinically supervise and line manage the Out of Hours Team ensuring staff adhere to the Trust values and the NMC code

To work with and deputise for the Service Lead as required

To engage and lead on caseload management by overseeing, monitoring and evaluating care plans for all patients to ensure quality care, treatment progression and timely discharge.

To provide overview of remote monitoring systems within Out of Hours and liase with UCCH and VW Hub to support service delivery

To support the Clinician of the Day (COD) with delegation of tasks and the prioritization of work based on clinical acuity.

To work within the integrated team to prevent unnecessary admission to hospital and facilitate early discharge form hospital.

To work with all health care professionals, and statutory/non-statutory agencies to provide a seamless, integrated service to our service users

To support community clinicians and students through supervision, appraisal and mentorship in line with Trust policy.

To oversee that clinical supervision is occurring within the team and accurately recorded.

To liaise with the Out of Hours Clinical Operations Manager (COM) to identify gaps in management and offer support as needed.
