Job summary

We are seeking an experienced and competent nurse with strong leadership and management experience to join our Central Community IV Therapy Team.

The Clinical Lead Manager will provide strategic, clinical and operational leadership for the Central Community IV Therapy Team, ensuring the delivery of safe, high-quality, patient-centred intravenous (IV) therapy within the community. The post holder will lead and support a skilled multidisciplinary team, promoting excellence in clinical practice, effective workforce management, service development and continuous improvement.

A key aspect of the role is to develop and maintain a strongpartnership with the local acute NHS Trust, working collaboratively across organisational boundaries to provide a coordinated pathway for patients requiring IV therapy outside of the acute hospital setting. The Clinical Lead Manager will act as an important link between community and acute services, supporting effective communication, shared clinical decision-making, timely escalation and safe transfer of care.

Main duties of the job

To provide professional and clinical leadership, demonstrating a supportive and flexible leadership approach and a proactive responsible attitude to promote confidence in the team and Service.

To clinically supervise and line manage the IV Therapy Nursing team ensuring staff adhere to the Trust values and the NMC code

To work with, and deputise for, the Service Lead as required

To engage and lead on caseload management by overseeing, monitoring and evaluating care plans for all patients to ensure quality care, treatment progression and timely discharge.

To review and develop trust policies and SOPs in relation to service development

Develop and oversee new clinical pathways to support service development and continuous improvement.

To work within the integrated team to prevent unnecessary admission to hospital and facilitate early discharge from the acute trust

To work with all health care professionals, and statutory/non-statutory agencies to provide a seamless, integrated service to our service users

To support community clinicians and students through supervision, appraisal and mentorship in line with Trust policy.

To oversee clinical supervision is occurring within the team and accurately recorded.
