Job summary
We are looking for a motivated, enthusiastic and innovative Band 4 therapy support worker to work with an inspiring multidisciplinary team; including Physiotherapists, Occupational Therapists (OT), Speech and Language therapist. The role will involve supporting patients following long hospital admissions, including time in critical care.
You will work in an outpatient and community setting undertaking a variety of duties with guidance and be responsible for an agreed caseload.
Training to develop competence in a variety of OT and Physiotherapy assessments/treatments will be provided. You will be key in supporting assessment and rehabilitation of patients presenting with symptoms of deconditioning, breathlessness and fatigue.
The role includes triaging of patients, therapeutic interventions, home visits, exercise group support. You will demonstrate an ability to work with patients with a wide range of conditions. Good clinical and interpersonal skills and multi-disciplinary team working are essential. The ability to travel independently between locations is essential.

Main duties of the job
The role includes triaging of patients, therapeutic interventions to aid improvement in mobility/return to functional activity, decreasing impact of fatigue on day to day life, managing breathlessness in patients, home visits, and exercise group support. Throughout all components supporting and motivating patients to engage and being mindful of the difficult time they have been through or chronicity of their conditions.
