Job summary

An exciting opportunity has arisen to join Manchester Neighbourhood Mental Health Team as an Advanced Clinical Practitioner.

The team is developing to meet the needs of people with complex and serious mental health challenges in a neighbourhood setting.

You will be supported by the MDT including staff from GMMH and VCSE organisations.

Main duties of the job

The Advanced Clinical Practitioner will work within a trauma informed model to engage and offer a specialist service to the service users referred to and under the care of the Manchester NMHT. This would include attendance at clinical multi-disciplinary team meetings, facilitating clinic appointments both out in the community and in a clinic setting, commencing and maintaining medication regimes, completing appropriate Paris documentation, and being a part of the Senior Leadership Team.

The ACP will be responsible for assessment, formulation and prescribing of care planning in line with the service Standard Operating Procedure and agreed protocols and pathways. The post holder will be recognised as an expert practitioner and will advise the Multi-Disciplinary team on expert, safe and effective models of care and treatment.

They will provide clinical leadership and management leading to the provision of safe, quality and effective care. They will also lead on Physical Healthcare agenda for the service and will support the improvement of workforce knowledge through education and training programme. They will support with the ongoing development of the service and developing and maintaining good relationships across the Manchester mental health system.

The post holder may additionally lead on practice and service developments. The post holder is required to demonstrate clinical and practice expertise via interventions, supervision and modelling good practice.
