Job summary

Are you a patient-focused Registered Nurse or Allied health professional looking for an exciting opportunity to develop your clinical triage skills within a new and evolving community service?

We are seeking a motivated and enthusiastic Band 6 Registered Nurse or Allied Health Professional to work within Care Connexions. This role offers an excellent opportunity to be part of an innovative service supporting timely, joined-up care for patients in the community, with a strong focus on hospital avoidance and ensuring people receive the right care, in the right place, at the right time. The service currently operates across 7 days, 08:00hrs - 20:00hrs

Main duties of the job

This exciting role will support the ongoing development of an integrated community response model, working in partnership with acute trusts, primary care, Northwest Ambulance Service, the voluntary and third sector, Social Care, therapy services and wider community teams.

The role will focus on clinical triage, ensuring referrals are responded to in a timely and effective way so that patients are directed to the most appropriate service. You will work closely with a Multi-disciplinary team consisting of Advanced Clinical Practitioners, Nurses, Therapists and Assistant practitioner to support safe, coordinated care planning.
