Job summary

This is an exciting Band 7 opportunity to clinically lead the Calderdale Post Covid service.

You will join a multiprofessional team who are passionate about the service they deliver. The team works with a strong emphasis on self management and delivers symptom based rehab. You will provide expert triage, assessment, treatment and clinical coordination of patients in CHFT's pathway.

The expectation is that this service will develop in line with West Yorkshire ICB recommendations to expand to include other conditions who would benefit from a similar symptom based approach. You will work alongside the service lead to develop this new vision and pathway.

You will also be leading, facilitating and working with a number of professions including direct liaison with primary care, referrers and consultants in secondary care.

This service is delivered in a variety of settings: classes, patient homes, clinic and virtual. You will support the training and skill acquisition within the core team but also support training needs of services across the Calderdale place. As a new and emerging service, you and the service lead will be responsible for the development and evaluation of the service.

The postholder will deliver a high quality specialist Post Covid service to CHFT 's Calderdale residents. They will take a specialist lead clinical role and will work as an integral part of the Post COVID pathway providing leadership, consultation and supervision to other professional colleagues.An important element of the role is the ongoing development of a newly emerging speciality, therefore knowledge in post infectious conditions their presentation and treatment is essential alongside service development.

  • To be responsible for planning, coordinating, delivering and evaluating clinical case work within the Post Covid service, to include triage, initial assessment and MDT working.
  • To provide evidence based, highly specialist knowledge, assessment and treatment skills to the Post Covid service
  • To be responsive to emerging evidence around post viral illnesses. Educating staff members and altering/adapting clinical pathways as appropriate.
  • To manage autonomously a clinical workload of patients with Post COVID and post infectious/viral conditions at a highly specialist level.
  • To support the Post Covid service lead in developing, establishing and evaluating the new emerging service and pathway.
  • To contribute to measuring the effectiveness of the service, collecting and analysing data, producing team reports and making recommendations.
  • To take responsibility for effective functioning of the overall team within the Post Covid pathway. This will require effective communication and direction of professionals from CRT, Pulmonary Rehab, Inpatient Therapy, Respiratory Care and Psychology.
  • To take a lead with quality improvements in the Post Covid service.
  • To support the training, education and development of the team, students and services across the system.
  • To lead the promotion of the service and encourage communication; education and joined up working, in particular with primary care and public health. Support partnership working with patients, peer support groups and local authority teams and voluntary services.
  • To participate in the supervision and appraisal process with staff and students as required.
