Job summary
To support the Clinical Nurse Specialist in the Haematology team to deliver excellent, personalised care to Haematology patients whilst developing your own skills and knowledge within the speciality.
Main duties of the job
To support patients in clinics alongside consultants- offering holistic care and support where necessary.
To follow up with patient post clinic to ensure continuity of support.
Managing the telephone enquiries of patients and their carers; prioritising, assessing and escalating as appropriate.
To support the Clinical Nurse Specialist in the preparation of the nurse led clinics liaising with primary care providers to ensure all relevant medical details are available for the clinics.
Job description
o Supporting the CNS and managing independently the care of patients with malignant and non-malignant haematological conditions working within your scope of practice.
o With supervision provide timely assessment and clear, evidence based clinical management plans for these patients.
o Develop through training and experience, specialist haematology knowledge to underpin evidence-based care.
o To act as a point of contact for patients and their families and facilitate access to specialist nursing care and information.
o Provide telephone support/advice for patients, carers and other health care professionals and liaise with the MDT to ensure high quality appropriate care and management for patients.
o Act as an advocate for the patient and facilitate their involvement in decisions affecting their care.
