Job summary
The Community Inpatient Unit provides Rehabilitation, Re-ablement and recovery interventions for adults in North East Lincolnshire. The service will promote faster recovery from illness, prevent unnecessary acute hospital admissions and premature admissions to long-term care, support timely discharge from hospital and maximise independent living wherever possible. Multi-professional staff will collaborate with individuals to ensure they achieve their full potential in terms of their level of independence, health, and wellbeing.
CIU provide short term reablement for Services Users who may need to regain their confidence and skills on discharge from hospital or to get over a short-term health exacerbation. This includes assessing complex and specialist needs and developing, implementing and evaluating programmes of care, in order to support individuals to remain in community settings, in their own home, residential home or nursing home.

Covering 7-day provision dependent on service needs
Salary Range 5 £30,805 - £35,715 per annum (Pay award pending)
We are seeking a highly motivated and passionate Band 5 Nurse to join the multidisciplinary team on a temporary basis at the Community Inpatient Unit (CIU), which provides nursing and enablement services to patients within the Rehabilitation and Recovery Inpatient Unit.
CIU provide short term reablement for Services Users who may need to regain their confidence and skills on discharge from hospital or to get over a short-term health exacerbation. This includes assessing complex and specialist needs and developing, implementing and evaluating programmes of care, in order to support individuals to remain in community settings, in their own home, residential home or nursing home.
The role will require you to undertake various clinical duties including assessment, wound management, injections, and continence care as well as dealing with acutely unwell. Ideally we are looking for some with previous community experience however this is not essential as you will received training and support.
