Job summary

We are seeking a dedicated Assistant Practitioner, or someone to develop into this role, to join our dynamic Community Stroke Team. The post holder will contribute to the delivery of specialist rehabilitation across Occupational Therapy, Speech and Language Therapy, and Physiotherapy. The role requires a flexible approach, with responsibilities spanning outpatient and community environments.

You will work with patients individually, in groups, and as part of a multidisciplinary team. Ongoing professional support and development opportunities will be provided to enhance your skills within this specialist area.

As community visits form a key part of the role, a full driving licence and access to a vehicle are essential.

Main duties of the job

Deliver treatment programmes that support patients in achieving their rehabilitation goals.

Undertake stroke and neurology-specific therapeutic interventions under the supervision of qualified therapists.

Act as a key worker for allocated patients, ensuring effective communication and feedback to clinicians.
