Job summary

We are seeking an enthusiastic, experienced, and highly motivated Advanced Nurse Practitioner (ANP) to join our friendly and supportive clinical team at Amersham Health Centre.

The successful candidate will be an autonomous practitioner, providing high-quality, patient-centred care within primary care. You will undertake advanced clinical assessment, diagnosis, treatment, and management of patients presenting with undifferentiated and undiagnosed conditions, as well as acute and chronic illnesses, across all age groups.

Main duties of the job

Key responsibilities include:

  • Undertaking comprehensive clinical assessments, formulating differential diagnoses, and implementing appropriate treatment plans.
  • Independently prescribing medication in accordance with current legislation and best practice.
  • Managing patients presenting with minor illness within your scope of practice.
  • Assessing, diagnosing, treating, and referring patients appropriately using evidence-based practice.
  • Supporting the management and review of patients with long-term conditions.
  • Providing health promotion, preventative care, and lifestyle advice to improve patient outcomes.
  • Requesting, interpreting, and acting upon appropriate investigations and diagnostic tests.
  • Working collaboratively with GPs, nurses, clinical pharmacists, and the wider multidisciplinary team to deliver safe, effective, and coordinated care.
  • Contributing to clinical governance, quality improvement initiatives, audit, and service development.
  • Supporting the continued development of practice services to enhance patient access, quality, and efficiency.
  • Maintaining accurate, contemporaneous clinical records in accordance with professional and organisational standards.

Job description

Clinical Practice

  • Undertake comprehensive history taking, clinical examination, assessment, diagnosis, treatment, and management of patients across all age groups.
  • Assess and manage patients presenting with undifferentiated illnesses and acute presentations.
  • Diagnose and manage minor illnesses within own competence.
  • Independently prescribe medication in accordance with current legislation and local prescribing guidance.
  • Request, interpret, and act upon laboratory and diagnostic investigations.
  • Formulate appropriate management plans and arrange follow-up where required.
  • Refer patients appropriately to secondary care or other healthcare professionals.
  • Recognise and respond appropriately to medical emergencies.
  • Maintain accurate, timely, and legally compliant clinical records using EMIS Web.
  • Work within professional competence and the NMC Code at all times.

Long-Term Condition Management

  • Support the assessment and management of patients with long-term conditions.
  • Deliver evidence-based care in line with national and local clinical guidelines.
  • Promote self-management and patient education.
  • Undertake appropriate medication reviews where clinically indicated.

Health Promotion

  • Promote healthy lifestyles and preventative healthcare.
  • Provide opportunistic health education and screening advice.
  • Support vaccination and disease prevention initiatives where appropriate.

Multidisciplinary Working

  • Work collaboratively with GP Partners, salaried GPs, nurses, healthcare assistants, pharmacists, pharmacy technicians, care coordinators, social prescribers, and the wider Primary Care Network.
  • Participate in multidisciplinary meetings where appropriate.
  • Support integrated patient care across community services.

Quality Improvement

  • Participate in clinical audit, service evaluation, and quality improvement projects.
  • Contribute to the development and implementation of practice protocols and clinical pathways.
  • Support the achievement of Quality and Outcomes Framework (QOF) and other contractual requirements.

Professional Responsibilities

  • Maintain professional registration and comply with NMC standards.
  • Participate in appraisal, mandatory training, and continuing professional development.
  • Keep clinical knowledge and prescribing practice up to date.
  • Act as a positive role model within the practice.
  • Maintain patient confidentiality and comply with GDPR, information governance, safeguarding, and infection prevention policies.

General Responsibilities

  • Work flexibly to meet the needs of the practice.
  • Participate in practice meetings and protected learning events.
  • Undertake any other duties appropriate to the role and level of responsibility.

Person SpecificationEssentialQualifications

  • Registered Nurse with current NMC registration.
  • Advanced Nurse Practitioner qualification (Master's level or equivalent).
  • Independent Prescriber (V300).
  • Recognised Minor Illness qualification.
  • Evidence of ongoing Continuing Professional Development.

Experience

  • Experience working as an Advanced Nurse Practitioner in General Practice or Primary Care.
  • Experience assessing patients with acute and undifferentiated conditions.
  • Experience independently prescribing.
  • Experience managing long-term conditions.
  • Experience using clinical computer systems (EMIS Web desirable).

Knowledge

  • Primary Care and NHS contractual requirements.
  • NICE guidance and evidence-based practice.
