Job summary

This is an exciting opportunity to join a well-established specialist community service within the Neurodiversity services in NE Lincolnshire to provide support for service users within the Forensic Outreach Liaison Service (FOLS).

The local NE Lincolnshire FOLS Team is part of a wider FOLS Service working beside HUMBER, which means you will also be working alongside Specialist Forensic Services in Hull to support our Transforming Care partners to repatriate offenders with learning disabilities (LD) and/or Autism and complex needs back to their local communities.

The aim of the FOLS is to reduce or safely manage behaviour amongst people with LD and/or autism which may put members of the public at risk and would thereby otherwise lead to contact with the criminal justice system or admission to a secure hospital.

The initial focus of the service will be to support an identified patient cohort transition effectively from secure care into community provision; safely managing risk and avoiding contact with the criminal justice system or re-admission to secure hospital. The service will support a more effective approach to the identification and management of individuals at risk of offending or being admitted into secure services.

Range 7 £45,987 - £52,434 (pay award pending)

We are looking for an experienced and enthusiastic professional to join our Forensic Outreach Liaison Service (FOLS)

You will:

  • Manage a specialist caseload, provide expert advice on risk assessment and management
  • Contribute to safe, effective, and person-centred care
  • Support and develop colleagues through training, consultation, and professional leadership, helping to enhance skills and confidence
  • Contribute to service development, quality improvement and the implementation of best practice
  • Help ensure positive outcomes for service users while promoting recovery, safety, and community inclusion

A relevant professional registration with good working knowledge of the Mental Health Act and health & social care pathways is essential as well as experience working with complex cases in the field of LD and/or Autism.

Good communication and organisation skills are also essential as well as the ability to work autonomously and as part of a Multi-Disciplinary Team working in partnership with other services to meet the Transforming Care targets and to provide a successful community based forensic support service that improves the health and wellbeing of local people.

Following the shortlisting process, successful applicants will be invited to deliver a formal presentation, which will be followed by a panel interview at the Val Waterhouse Centre, Grimsby.
