Job summary

The post holder will work as part of the Primary Intermediate Mental Health Team across the Blackpool locality. The role focuses on delivering high-quality clinical care, including supporting patients to access and attend annual physical health checks and, where appropriate, directly undertaking these checks (e.g. Severe Mental Illness physical health checks), supporting patients with long-term conditions, and contributing to multidisciplinary community services.

Main duties of the job

The post holder will manage their own caseload, clinics, and appointment systems, follow agreed care plans, monitor patients' ongoing physical health, and maintain a high level of clinical competence. The post holder will support the Localities team in supporting patients complete interventions. Responsibility for continuous professional development, mandatory training, and adherence to Trust policies is integral to the role.

Job description

  • Undertake delegated clinical duties within the clinical setting.
  • To maintain the highest possible standard of clinical practice
  • To supervise other practitioners within the team and develop their appropriate knowledge and skills.
  • To be involved in the delivering care to patients with long term conditions.
  • To assist the Registered staff in the interpretation and analysis of clinical and non-clinical facts to inform accurate treatment in a wide range of conditions and to assist in the recommendation of the best course of intervention and to assist in the development of discharge plans.
  • To work as part of the multi-disciplinary team, referring to other staff members/ services where appropriate.
  • To help establish and maintain communication flows between the Trust and other health and social care providers regarding ongoing care.
  • To assist in the evaluation of new equipment as appropriate. To be responsible for all the equipment used in carrying out duties and adhere to hospital policies, including competence to use the equipment.
  • Have an understanding of the roles of the physiotherapist and Occupational therapist and where appropriate be involved in the assessments and be able to complete referrals to both disciplines.
  • Extended communication skills to advise patients of care and treatments.
  • Assist with patient referral and discharge paperwork, take and record clinical observations.
  • To develop skills in identified extended roles such as Venepuncture.
  • Have knowledge of Trust policies and procedures.
  • The ability to utilise resources effectively.
  • Develop the skills required to manage a team of patients under the guidance of the Registered Practitioner.
  • To liaise closely with the dietetic services and complete referrals.
  • To refer patients appropriately to Specialist Practitioners and to work closely with them.
  • Carry out home visits to patients in accordance with the Trusts Lone Working Guidance.
  • To closely work with external agencies to support patients attending annual physical health checks, or where appropriate, directly undertaking these physical health checks.
  • To provide support to PIMHT medical/ prescribing team for required physical health monitoring following commencement of prescribed medication.
