We are seeking an enthusiastic and committed person to join our Community Nursing Cullompton Community Nurse Team as a Band 5 Registered Nurse for. This is an exciting and new opportunity to work jointly across mixed of rural and urban community nursing. As a Registered Nurse in the community, you will be expected to provide Nursing Support, Assessment and Oversight to our patients in a wide range of clinical community environments across the Cullompton, Dunkeswell and Blackdown Hills area in the comfort of their own homes. Set in a Rural and Market Town location, you will gain a unique experience in working in Community nursing, and develop a skill set that is transferable across both Intermediate Care, Proactive care and Admission Avoidance.

Along with supporting MDT clinicians with their patient workloads delivering clinical care as part of both the admission avoidance and early supported discharge process, you will plan and evaluate and deliver patient centred care looking at proactive care as part of long-term conditions management. The post holder will be part of an integrated multi-disciplinary team of both primary and secondary care consisting of GPs, Nursing staff, OTs, Physios, and Community Matrons, who are responsible for the planning, implementation and evaluation of the patients needs and the delivery of high-quality care. You will be supported in your role by an Operational Management Team; Clinical Leads and clinicians with specialised skills. This role involves both acute illness and chronic condition management, within the patients own home. As part of this you will be working alongside support workers, and this role will involve supervisory and development work for those staff at Band 3 and Band 4 level.

The team have the use of shared pool cars to facilitate transport to more rural areas and the post holder will need to hold a driving licence.

Working Pattern:

Role 1) 22.5hrs - Substantive permanent

Role 2) 28hrs - Fixed term maternity cover until 17/10/2027
