Job summary
The Proactive Neighbourhood Care and Remote Monitoring service will support people living with frailty, complex needs and those who are housebound. The service will combine proactive home visiting, care coordination, remote monitoring and links into wider neighbourhood and Local Authority support.
Drivers will support the safe and efficient delivery of home visiting by transporting clinicians and, where agreed, equipment across Berkshire West. We would like to build a wider bank while identifying two reliable drivers who are available for regular ongoing work.
Main duties of the job
- Transport clinicians safely and punctually between patients homes and agreed service bases.
- Plan routes and respond flexibly to changes in the visiting schedule.
- Keep the vehicle clean, roadworthy and appropriately insured for business use.
- Support the safe transport of small items of clinical equipment where required.
- Maintain patient confidentiality and professional boundaries at all times.
- Communicate delays, incidents or concerns promptly to the service team.
- Complete any required induction, safeguarding, information governance and lone-working training.
