Mental Health Social Care Team
Permanent, Full-time
Hours per week: 37 hours
Salary: £31,537 - £33,699 per annum
Main duties of the job
We are looking for a compassionate and skilled practitioner to join our Mental Health Social Care Team, supporting people of all ages across Wiltshire. You will work with adults, and older people, including those with learning disabilities, who meet the threshold for secondary mental health care.
This is a varied and rewarding role where you will carry out assessments, manage care plans, and work closely with health professionals to ensure people receive the right support at the right time. You will help individuals maintain independence, manage risk, and access services that meet their needs, while ensuring their experience of health and social care is as seamless and integrated as possible.
You will be part of a multi-disciplinary team, liaising with our partners at AWP and other adult social care teams. You will also support safeguarding, carers assessments, and contribute to service development by listening to feedback and identifying improvements.
We are looking for someone with experience in health or social care, a good understanding of mental health, and a commitment to putting people at the heart of everything we do.
