Job summary

The Families' Health and Wellbeing Service (0-19) in Staffordshire have a vacancy for a forward-thinking Band 6 Health Visitor, to work within our unique developing Children's and Families Single Point of Access Hub based in Rugeley. The hub provides a gateway to the Families' Health and Wellbeing Service, utilising new and innovative technology to reach out to families and young people to provide advice and support.

We are seeking a person who will be caring and compassionate, who can actively listen and have a warm approachable manner. You will lead by example and encourage effective skill mix team working to develop others and ensure the best outcomes for our Children and families.You will be communicating with our universal and strengthening families' teams across the districts, supporting the 0-19 Healthy Child Programme for a varied, unique and diverse population. You will also work closely with partner agencies across health, social care and the local authority.

The successful applicant must have the ability to travel within the Trust

Main duties of the job

  • Becompetentasappropriate totherequirementsofthe role.
  • Workwithintheirscope ofpractice
  • DemonstrateatalltimestheValuesofMidlands Partnership NHS Foundation Trust.
  • Be a Specialist Community Public Health Practitioner and have a detailed professional and clinical knowledge.
  • Be responsible for a defined population of service users in a variety of settings across the Partnership Trust including the community and hospitals.
  • Be skilled in the assessment of children and young people's care/ support needs and in the development, implementation and evaluation of care/support packages.
  • Have supervisory, training and leadership responsibilities where appropriate and will have some responsibility for team performance and service developments/improvements.
  • Demonstrateinitiativeandbecreativein findingsolutionstoproblems.
  • Workwith theteamleaderundertakingdelegatedtasksin teammanagement
  • To deliver commissioned aspects of the Healthy Child Programme, ensuring key objectives and outcomes are achieved.
