We are seeking a dedicated and compassionate Learning Disabilities Nurse to deliver specialist learning disability and mental health support within a prison setting. The successful candidate will manage a varied caseload of service users with learning disabilities and complex needs, undertaking assessments, developing care plans, and delivering person-centred interventions that promote wellbeing and positive outcomes.

Working as part of a multidisciplinary and multi-agency team, the post holder will collaborate closely with prison-based mental health services and a range of internal and external stakeholders. The role involves providing expert advice, guidance, and support to colleagues and partner agencies, ensuring service users receive high-quality, coordinated care tailored to their individual needs.

Main duties of the job

As a key member of a multidisciplinary team, you will manage your own diverse caseload, undertaking assessments, and monitoring and delivering specialist nursing interventions to individuals with learning disabilities and co-occurring mental health needs. You will play a vital role in supporting service users throughout their journey, ensuring care is person-centred and responsive to their changing needs.

Working collaboratively with healthcare professionals, prison staff, and other agencies, you will provide expert learning disability advice, contribute to multidisciplinary meetings and case reviews, and champion the needs of some of the most vulnerable individuals within the criminal justice system.

Responsible for the effective utilisation of clinical resources to ensure adequate clinical care is provided with a requirement that all incidents, which may compromise care, are reported immediately to a senior manager, and clinicians involved in their care as appropriate.

A key aspect of the role is ensuring continuity of care and successful transition planning, coordinating services and aftercare arrangements to support individuals both during custody and upon release. You will work within the Care Programme Approach framework, helping to create seamless pathways between prison and community mental health and learning disabilities services.

Line management of staff where appropriate

Be familiar with and ensure implementation of all local, regional and national policies, procedures and guidelines pertinent to all staff within area of responsibility and comply with their contents in the pursuit of the highest standards of care for service users.

To provide a range of interventions in line with the learning disability pathway across the integrated offender health model

To support and liaise with the substance misuse team, primary/secondary care teams and to deliver learning disability interventions.

Act upon referrals from prison based primary care staff, and external teams. Referrals are allocated via the team meeting. The post holder will provide assessment, implementation and development of care programmes for individuals on their caseload.

To be aware of polices relating to vulnerable adults, mental capacity and learning disability, to be able to work within this framework.
