Job summary

The Community Urgent Care Team (CUCT) provides a comprehensive 24/7 health and social care triage service to patients and/or their carers/relatives with the aim of averting the need for hospital admission or early hospital discharge.

This includes assessing complex and specialist needs in order to develop, implement and evaluate the appropriate programmes of care and wherever possible, supporting individuals to remain in a community setting whether it be their own home, a residential home or nursing home. The CUCT will continue to be involved with the individuals programme of care for up to 7 days if required.

37.5 hours a week, Permanent

Working on a rota of 12 hour shifts; including weekdays, nights, weekends and bank holidays

Salary Range 6 £36,806-£44,717 pro rata per annum. Pay award pending.

The post holder will assess complex and specialist needs in order to develop, implement and evaluate programmes of care and supporting individuals to remain in community settings, their own home, residential or nursing home. Providing a first point of contact/triage for patients with undifferentiated and undiagnosed health problems. Working predominantly in an autonomous and independent manner and will assess, exam diagnosis and implement a plan of treatment which may include follow up, discharge or referral to other health care professionals.

This exciting and challenging role offers the opportunity to provide high quality patient focused clinical care in the community. You will be responsible for health assessments including exacerbations, undifferentiated and undiagnosed of acute and chronic conditions. You will develop an understanding and application of social assessments in crisis. Good communication skills are essential to support collaborative working within the team and alternative agencies within health and social care. Community experience is desirable, although training can be offered to those with a wide range of transferable skills and experience.
