Main duties of the job

We are looking for a compassionate and autonomous clinician to help deliver high-quality care to some of our most vulnerable patients within the community. This rewarding role focuses on supporting housebound, frail, elderly and clinically complex patients in their own homes, helping them maintain independence and quality of life.

You will be part of a friendly and supportive multidisciplinary team, working closely with GPs, nurses, care coordinators, care homes and community services to provide personalised, joined-up care. The role offers a varied caseload and the opportunity to build meaningful relationships with patients and their families whilst making a genuine difference to their health and wellbeing.

The successful candidate will contribute to proactive and preventative healthcare, supporting patients with long-term conditions, frailty and complex care needs. There will also be opportunities to support palliative and end-of-life care, advance care planning and community vaccination programmes for eligible housebound patients.

Job description

The Visiting Clinician will play an important role in delivering high-quality, patient-centred care to patients within their own homes, with a particular focus on housebound, frail, vulnerable and clinically complex individuals. The role is designed to support patients who may otherwise struggle to access care, helping them maintain independence, improve wellbeing and receive the right care at the right time.

Working as part of a friendly and supportive multidisciplinary team, you will undertake a varied caseload of home visits, providing assessment, diagnosis, treatment and ongoing management for patients with a wide range of healthcare needs. You will work closely with GPs, nursing colleagues, care coordinators, community teams, care homes and other healthcare professionals to ensure seamless, coordinated care for patients across our population.

The role offers the opportunity to make a real difference through proactive and preventative healthcare. This includes supporting patients living with frailty, multiple long-term conditions and complex health needs, identifying those at risk of deterioration and helping prevent avoidable hospital admissions through timely intervention and personalised care planning.

The successful candidate will also have the opportunity to contribute to palliative and end-of-life care, advance care planning and vaccination programmes for eligible housebound patients. You will work in partnership with patients, families and carers to ensure that care is delivered with compassion, dignity and respect, supporting people to achieve the best possible outcomes in a setting that is often most comfortable and familiar to them.

This position offers a high degree of autonomy whilst maintaining the support of an experienced and approachable clinical team.
