Job summary
We are looking for an experienced and motivated Practice Nurse to join our friendly and supportive primary care team. This is an exciting opportunity to work in a well-established practice committed to delivering high-quality and patient-centred care.
The successful candidate will play a key role in the management of long-term conditions alongside providing general practice nursing services, including cervical screening, immunisations, wound care and health promotion. Independent chronic disease management experience is essential.

Main duties of the job
The main focus of this role will be the delivery of high-quality nursing care, including the assessment, monitoring and management of patients with long-term conditions. The successful candidate will support patients in managing their health conditions, contribute to improving long-term health outcomes within the practice and provide patient education to promote wellbeing and self-care.
In addition to long-term condition management, the Practice Nurse will provide a range of general practice nursing services, including chronic disease management, cervical cytology, immunisations, wound care, health promotion and screening activities. The role will involve working closely with the wider multidisciplinary team to deliver safe, effective and patient-centred care.
We are looking for an experienced Practice Nurse who can work independently, prioritise workload effectively and build strong relationships with patients and colleagues. Experience in primary care is essential, and a proactive approach to service improvement and patient education would be highly valued.
