We are looking for an experienced Primary Care Nurse, newly qualified and secondary care may apply with the understanding there will be further training required, thank you.

We are seeking a compassionate and proactive registered Nurse to join our team and to support the delivery of high-quality patient care within our busy GP surgery.

The successful candidate will be working across our 3 sites with our varied Nursing Team. Primary care experience is essential.

Main duties of the job

This will be a varied role including nursing clinics.

Providing an excellent patient service and patient relationship skills, excellent communication skills, a confident manner and the ability to remain calm under pressure are essential attributes of any applicants.

Job description

Working in accordance with the UKCC Code of Conduct, the post-holder will support and develop the Practice nursing team in the provision of assessment, treatment, screening, health education services and advice to patients.

The post-holder will agree protocols and guidelines to provide nursing treatments to patients both independently and in participation with nurse colleagues and general practitioners.

The post-holder will have the required level of training and competence and will work within patient group directions where these are available. A nurse with prescribing qualifications will use them appropriately.

Clinical Responsibilities:

  • Identify, assess and diagnose patients with complex health needs across the Practice population.
  • Work with others to change policies to improve health and well-being and reduce inequalities.
  • Assist with the implementation and evaluation of evidence-based practice to improve the services offered by the Practice nursing team.
  • Assist with management of Chronic Diseases, wound care, health screening, travel health, health screening, immunisations (Adult and Child), Cytology.
  • Stock maintenance and ordering.
  • Any other delegated duties appropriate to the post.

  • The post-holder will implement and assist with a full range of promotion and management of their own and others health and safety and infection control as defined in the practice Health & Safety Policy, the practice Health & Safety Manual, and the practice Infection Control policy and published procedures.

  • In addition to maintaining continued education through attendance at any courses and/or study days necessary to ensure that NMC professional development requirements are met, the post-holder will participate in any training programme implemented by the Practice as part of this employment.

  • Alert other team members to issues of quality and risk.
  • Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the teams performance.
  • Work effectively with individuals in other agencies to meet patients needs.

Communication:

  • The post-holder should recognise the importance of effective communication within the team and will strive to communicate effectively with other team members, patients and carers. Recognise peoples needs for alternative methods of communication and respond accordingly.

Qualifications required:

  • First level nurse, relevant nursing/health degree.
  • Professional registration with the NMC - Nursing & Midwifery Council.
  • Cervical Screening, Chronic Disease Management, Baby Immunisations.

Personal attributes:

  • Excellent communication skills, highly motivated, flexible, enthusiastic, attention to detail, team player.
