From the day of discharge, our dedicated team of Home First Support Workers, Assessors and Team Leads provide early intervention, personalised support and seamless coordination with health, social care and voluntary sector partners.

We take time to understand each person's unique situation, strengths and goals, creating tailored plans that may include care support, specialist equipment or connections to community organisations such as Age UK. By focusing on what matters most to individuals, we help people recover faster, reduce reliance on hospital-based care and achieve greater independence.

Main duties of the job

As part of the Home First discharge model, the Discharge to Assess (D2A) team helps people move safely from hospital to the most appropriate setting.

You'll oversee the delivery of a high-quality administrative service that directly supports patient flow and better outcomes for people across Kent. You'll lead and motivate a dedicated administration team, ensuring work is coordinated effectively and service standards remain high.

No two days are the same. You'll be responsible for line managing staff, conducting appraisals, supporting attendance and performance, developing and improving administrative processes, allocating workloads, analysing and collating data, and managing complex spreadsheets. You'll also work closely with health and social care professionals, provider services and system partners.

We're looking for a proactive, organised and people-focused leader who thrives in a fast-paced environment. If you're an excellent communicator, enjoy problem-solving, and are passionate about making a difference behind the scenes of patient care.

This hybrid role is based at Maidstone or Tunbridge Wells Hospital, with opportunities to work from home.

Additional Working Pattern: Monday - Friday 7.5 hours per day
