Main duties of the job

Your responsibilities will include:

  • Providing appropriate care and services to patients across all age ranges and levels of healthcare need.
  • Undertaking face-to-face consultations, telephone consultations, triage and home visits as clinically appropriate.
  • Monitoring the central advice pool across our services, where appropriate, to support timely clinical decision-making and patient safety.
  • Maintaining high-quality, accurate and up-to-date medical records, including appropriate documentation and clinical coding to support consistent RCGP audit requirements.
  • Providing clinical advice, guidance and support to other members of the multidisciplinary team when required.
  • Supporting our Clinical Practitioner workforce in the assessment and management of more complex cases.
  • Developing and maintaining effective communication with patients and carers to support safe, person-centred care.
  • Maintaining effective communication with acute, community and other health and care partners to support continuity and coordination of care.
  • Remaining vigilant to potential safeguarding concerns involving children and adults and taking appropriate action in accordance with statutory requirements
  • Contributing to clinical governance, quality improvement, learning
  • Working collaboratively as part of our wider multidisciplinary team to ensure safe, effective and responsive patient care.

Job description

Key Responsibilities

1. Clinical Care

The post-holder will

Provide timely, safe and effective clinical care to patients across all age ranges and levels of healthcare need.

Undertake comprehensive history taking, clinical examination and assessment.

Formulate differential diagnoses and evidence-based management plans.

Manage patients presenting with acute, chronic, complex, undifferentiated and undiagnosed conditions.

Undertake face-to-face, telephone and digital consultations as clinically appropriate.

Participate in clinical triage and same-day assessment.

Undertake clinically appropriate home visits.

Request, interpret and act upon appropriate investigations.

Prescribe safely and effectively within professional competence and current guidance.

Recognise and manage clinical deterioration, risk and diagnostic uncertainty.

Refer patients appropriately to secondary care, community services and other relevant providers.

Provide health promotion, disease prevention and self-management advice.

Support patients to make informed decisions about their care.

Maintain professional responsibility for clinical decision-making and patient management.

2. Same Day Access Provision

The post-holder will

Assess and manage patients presenting with urgent and acute healthcare needs.

Prioritise clinical workload according to patient need and risk.

Support appropriate admission avoidance where clinically safe.

Recognise patients requiring escalation to urgent or secondary care.

Support safe discharge and continuity of care following acute episodes.

Monitor the central advice pool across services where appropriate to support patient safety and timely clinical decision-making.

Provide clinical advice and support to other members of the multidisciplinary team.

Support Clinical Practitioners with more complex clinical cases where required.

3. Long-Term Conditions and Population Health

The post-holder will

Provide ongoing clinical management of patients with long-term conditions.

Manage patients with multimorbidity and complex healthcare needs.

Contribute to personalised care planning and proactive care.

Support preventative healthcare and population health initiatives.

Identify patients who may benefit from earlier or more proactive intervention.

Contribute to the reduction of health inequalities.

Support appropriate management of conditions including diabetes, cardiovascular disease, hypertension, heart failure, respiratory disease, chronic kidney disease, mental health conditions, frailty and dementia.

4. Clinical Records and Information Management

The post-holder will

Maintain accurate, contemporaneous and legally compliant clinical records.

Ensure appropriate documentation and clinical coding is used consistently.

Use SNOMED coding appropriately.

Use EMIS and associated clinical systems effectively.

Manage clinical correspondence, test results and workflow appropriately.

Support call and recall systems where relevant.

Maintain patient confidentiality and comply with information governance requirements.

Participate in digital models of care where appropriate.

Use clinical systems and technology effectively to support safe and efficient service delivery.

5. Multidisciplinary and Integrated Working

The post-holder will

Work collaboratively with GPs, Advanced Clinical Practitioners, Clinical Pharmacists, Practice Nurses, Healthcare Assistants, Care Co-ordinators and wider clinical and administrative colleagues.

Participate in multidisciplinary team working and case discussion.

Provide clinical advice and support to colleagues where appropriate.

Coordinate care for patients with complex needs.

Work effectively with community nursing, mental health, social care, allied health professional and secondary care teams.

Support integrated care planning.

Maintain effective communication with acute, community and other health and care partners.

Promote shared decision-making with patients, carers and families.

Support continuity of care across organisational boundaries.

6. Clinical Governance and Patient Safety

The post-holder will

Work in accordance with clinical governance and patient safety requirements.

Identify and escalate concerns relating to quality, safety and risk.

Participate in clinical audit.

Participate in significant event reviews and learning processes.

Contribute to quality improvement initiatives.

Apply current evidence, national guidance and local pathways to clinical practice.

Support implementation of service developments and agreed clinical standards.

Contribute to service evaluation.

Reflect on own performance and professional practice.

Manage clinical workload, time and resources effectively.

Contribute to a positive safety, learning and improvement culture.

7. Education, Training & Workforce Development

The post-holder will

Maintain professional competence through continuing professional development.

Participate in annual appraisal and revalidation requirements.

Maintain all required statutory and mandatory training.

Participate in clinical supervision and peer learning.

Contribute to education and training within the practice where appropriate.

Support the development of medical students, GP trainees and other healthcare professionals where relevant.

Share learning arising from audit, incidents, quality improvement and clinical practice.

Contribute to the development of multidisciplinary team capability.

8. Safeguarding

The post-holder will

Remain alert to safeguarding concerns involving children, young people and adults at risk.

Recognise, document and escalate safeguarding concerns appropriately.

Work in accordance with local safeguarding policies and statutory requirements.

Participate in safeguarding discussions and multidisciplinary processes where required.

Understand the needs of vulnerable patient groups.

Maintain appropriate professional curiosity and clinical vigilance.

Contribute to a culture in which safeguarding is recognised as the responsibility of all staff.

9. Communication

The post-holder will

Communicate effectively with patients, carers and colleagues.

Maintain a clear, professional and respectful approach to verbal and written communication.

Promote effective communication across the multidisciplinary team.

Support effective handover and continuity of care.

Ensure information is shared appropriately and in accordance with confidentiality and information governance requirements.

10. Professional Standards

The post-holder will

Work within GMC professional standards and relevant legal and regulatory requirements.

Maintain appropriate professional indemnity.

Maintain inclusion on the General Practitioner Register and National Performers List.

Maintain up-to-date revalidation and appraisal.

Recognise the limits of own competence and seek advice or support when required.

Take responsibility for professional conduct, clinical decisions and ongoing development.

Work in accordance with Marlow Medical Group policies and procedures.
