Out of Hours Clinical Team Leader

Band 7 | Fixed Term-12 months | Wokingham

Are you an experienced Community Nurse with strong leadership skills and a passion for delivering high-quality care? We are looking for an enthusiastic and motivated Clinical Team Leader to join our busy and rewarding Berkshire Out of Hours Community Nursing Service.

Working Monday to Friday, 15:00-23:00, with occasional weekend working, you will play a key role in the day-to-day running of the service, providing clinical leadership and supporting our nursing teams across Berkshire.

You will need:

  • Community Nursing experience at Band 6 level
  • Previous leadership and team management experience
  • A UK driving licence and access to a vehicle, as some locations are not accessible by public transport
  • Excellent communication and interpersonal skills, including confidence in managing challenging conversations
  • Experience of change management, service improvement or introducing innovative practice
  • The ability to support, develop and motivate staff to deliver safe, effective and evidence-based care

You will work closely with Senior OOH Nurses to ensure staff are supported, appropriately inducted and competent in their roles. You will help promote proactive community care that prevents avoidable hospital admissions and supports timely discharge.

Main duties of the job

  • Day to day management of the Berkshire Out of Hours Community Nursing service.
  • Exercise leadership and judgment in the management of Berkshire Out of Hours Community Nursing Team to ensure good clinical practice and maintain high standards of care, and review staff practice at regular intervals.
  • Responsible for the direct delivery and operational management of the service, developing rosters of staff from exiting teams to prevent inappropriate or unscheduled hospital admissions and facilitate timely discharge, across Berkshire.
  • Responsible for capacity planning and management for a seven-day service and liaison with other Team Leaders to ensure equity of staffing and workload levels to meet the needs of the service.
  • Assess current treatment practices, maximise cost effectiveness whilst improving outcomes.
  • Operationally manage a group of staff, including appraisal and personal development planning review and delivery, performance management, leave and sickness absence.
  • Supporting staff to develop and enhance their capabilities.
  • Supporting colleagues in the delivery of compassionate, safe and structured care.
  • Work innovatively to ensure Community Service is fit for challenges and future demands.
  • I.T. literate to support staff transitioning to mobile working.
