Diabetic Nurse General PracticeThe Diabetic Nurse will provide high-quality, patient-centred care to patients living with diabetes within the general practice setting. The role involves supporting patients to effectively manage their condition, prevent complications and improve their overall health.
Key responsibilities include:
- Conducting routine diabetes reviews and assessments.
- Monitoring and managing blood glucose/HbA1c, blood pressure, weight and cardiovascular risk.
- Supporting patients with lifestyle changes, diet, exercise and self-management.
- Reviewing and supporting diabetes medication and treatment plans, within scope of practice.
- Carrying out diabetic foot checks and identifying potential complications.
- Arranging and/or monitoring relevant blood tests and investigations.
- Providing education and support around insulin, blood glucose monitoring and medication where appropriate.
- Identifying patients requiring further clinical review or referral.
- Working closely with GPs, pharmacists, healthcare assistants and other members of the multidisciplinary team.
- Maintaining accurate clinical records and ensuring care meets current diabetes guidelines and practice requirements.
- Supporting the practice with diabetes QOF and enhanced service targets where applicable.

Diabetes Practice Nurse Job OverviewWe are looking for an enthusiastic and experienced Diabetes Practice Nurse to join our friendly, patient-centred team at Hersham Surgery.
The role will involve providing high-quality diabetes care, including annual reviews, diabetic foot checks, blood pressure and cardiovascular risk assessment, lifestyle advice, medication support and ongoing management of patients with Type 1 and Type 2 diabetes.
You will work closely with our GPs, nursing team and wider multidisciplinary team to support patients in managing their condition and achieving the best possible health outcomes.
We are looking for someone who:
- Is a Registered Nurse with relevant diabetes experience/qualification
- Has excellent clinical and communication skills
- Is confident working autonomously within their scope of practice
- Is passionate about patient education and preventative care
- Works well as part of a friendly and supportive team
