Salary: AfC Band 6

Hours: 37.5 hours per week. Including weekends and public holidays

An exciting opportunity has arisen to join our WEOLCCS team as a Lead Nurse. Would you enjoy a role working within a team that makes a real difference?

This role plays a vital part in the goal of the end-of-life care system in Wandsworth, supporting patients to achieve their preferred place of care and death, to avoid unplanned and inappropriate hospital admissions and to support rapid discharges from hospital.

The nature of the role will require you to use your initiative and will involve interactions and managing the wide range of complexities presented by patients, their carers and families.

Main duties of the job

  • To be responsible for all aspects of the day-to-day operational Leadership and management for a team of Royal Trinity Carers and Care Coordinators.
  • To act as an End-of-life Care champion, build and maintain professional relations (internally and externally) to promote service.
  • To work closely with all service delivery teams, Continuing Health Care and care agencies to ensure care is planned to meet the unique and changing needs of patients, their families, and friends.
  • To provide an expert clinical resource to the team, the organisation, and external stakeholders.
  • To support the Head of Service in developing and implementing strategic goals and direction.

Job description

The WEoLCCS provides a dedicated, specialised care coordination service and central point of contact for patients likely to be in their last year of life and those who care for them(formally and informally).

The main responsibilities of the Lead Nurse are:

  • To act as a community nursing end of life champion and inspire others demonstrating a range of skills
  • To directly manage and lead the Services Health & Personal Care Delivery Team
  • To provide clinical support to members of the Service
  • To work closely with the Services care delivery team, Continuing Health Care, and the care agencies to ensure care is planned to meet the unique and changing needs of the patients, their families and friends
  • To build and maintain professional relationships (internally and externally) and to promote Trinitys services
  • To work flexibly within the Clinical Hub Services team when there is a need
