You will use your expertise to support evidence-based prescribing decisions, optimise treatment outcomes, and enhance medicine safety. Your role will also involve promoting safe and effective medicine use, identifying and addressing unmet pharmaceutical needs, and contributing to patient care across the healthcare pathway. You'll also be responsible for clinical medication reviews, providing prescribing advice, and supporting the safe and timely patient discharge through effective medication reconciliation. You'll be required to contribute to service development, engage directly with patients and carers, and provide training on medicines optimisation.

Working Hours & Location

This role is based atSwindon Intermediate Care Centre, a modern healthcare facility providing highquality intermediate and inpatient care. The post is contracted at 18.75 hours per week, offering a flexible opportunity to deliver meaningful clinical impact within a supportive care environment.

Main Responsibilities

  • Deliver comprehensive clinical medication reviews for inpatients, optimising treatment and improving patient outcomes.
  • Provide expert prescribing, therapeutics, and medicinesrelated advice to clinical and nonclinical colleagues, including doctors, nurses, nursing associates, pharmacy technicians, and wider health and social care professionals.
  • Lead on medicines optimisation, supporting patients to gain the maximum benefit from their medicines.
  • Actively promote the safe, effective, and costconscious use of medicines in line with best practice and local formulary guidance.
  • Identify and address unmet pharmaceutical needs, ensuring high standards of medicines safety and governance.
  • Undertake deprescribing activities, reducing unnecessary medicines and minimising pharmaceutical waste.
  • Provide professional leadership on the ward for medicinesrelated matters, including risk management, clinical governance, formulary compliance, service development, and expenditure awareness.
  • Support safe and timely patient discharge through effective medicines reconciliation and coordination with the wider care team.
  • Deliver highquality, patientfacing care, engaging directly with patients and carers to support understanding and adherence.
  • Contribute to the development of the workforce by providing education and training on medicines optimisation to wardbased staff.
