Main duties of the job
IOS provides short-term, intensive, psychosocial support to people being discharged from AWP Intensive Services to their GP. As a Recovery Navigator, you will provide support that is: Short-term (up to 6 weeks) Intensive (2-3x per week) Person-centred 1:1 Outreach Who does IOS support? Individuals being discharged from AWP Intensive Services (IS) back to GP-led care Have remaining acute symptoms of risk or distress Would benefit from an extension of intensive support, but no longer require clinical intervention

Hold a small caseload of up to 6 clients Work collaboratively with individuals to understand their experiences, strengths, challenges, and risks Work in collaboration with intensive teams in AWP Contribute to the development of client formulations Create client support plans, and safety plans Deliver psychoeducation and skills-focused interventions Deliver support which is focused on reducing: oSuicidality oself-harm oAcuity of mental health symptoms
