Job summary

East and Central Brighton Primary Care Network consists of nine GP surgeries generally covering the Whitehawk, Queens Park, Kemptown, Hanover, Woodingdean and Lewes Road areas of Brighton.

The PCN Social Prescribing team will comprise of three full-time and two part time social prescribers.

The Role's Main Purpose

To provide personalised support to individuals, their families and carers to take control of their well-being, live independently and improve their health outcomes; strengthen personal resilience and reduce health inequalities by addressing the wider determinants of health such as debt, poor housing, physical inactivity and low understanding of the health condition.

To take a holistic approach, based on the persons priorities and the wider determinants of health to co-produce a personal action plan to improve health and wellbeing, introducing or reconnecting people to community groups and statutory services.

To encourage the patient to carry out the goals set in the plan and try to ensure the participants get any extra support they require to achieve their goals.

The role will require managing and prioritising an individual caseload, in accordance with the needs, priorities and any urgent support required by individuals.

Main duties of the job

Be the first point of contact for GP surgeries where social prescribing services will be delivered and manage referrals received.

Undertake person-centred, guided conversations with patients, developing tailored personal action plans as appropriate. Ensure that identified goals within the personal action plans are achieved.

Maintain contact with the participants throughout the duration of the project, stepping up and stepping down support as required and appropriate.Support participants to improve their confidence, self-esteem and be an active participant in the choices they make in their lives that impact on their health and wellbeing.

Work as part of a Multi-Disciplinary Team to ensure an integrated and personalised approach.

Carry out duties and responsibilities in compliance with Safeguarding and Health and Safety Policies.

Explore and build social network, offering and developing a range of relevant activities to meet people's needs and wishes.

Participate as needed in any local, regional and national workshops, learning events and other meetings, as per the requirements of the social prescribing team.

Attend and participate in regular supervision, appraisals, training and other internal meetings.
