Job summary

As a Registered Nurse at Stonewood, you will provide expert, person-centred clinical care while overseeing the delivery of safe, high-quality services for clients and supporting carers in the community.

Main duties of the job

  • Delivering clinical leadership to ensure safe, effective, person-centred care.
  • Developing, reviewing and updating personalised care plans, medication records and clinical risk assessments.
  • Undertaking initial client assessments, home reviews and ongoing clinical evaluations to meet changing care needs.
  • Providing clinical advice, guidance and support to carers, clients and their families.
  • Delivering and assessing clinical training, including medication administration, moving and handling, Care Certificate competencies and specialist skills.
  • Completing competency assessments, clinical audits and contributing to quality improvement.
  • Working collaboratively with GPs, hospitals, district nurses, safeguarding leads and the wider multidisciplinary team to coordinate care.
  • Maintaining accurate clinical records, managing incidents and escalating safeguarding or clinical concerns.
  • Participating in the clinical on-call rota, providing out-of-hours advice, crisis management and care escalations.
  • Maintaining active NMC registration and practising in line with the NMC Code.
  • Using digital care systems
  • Managing a varied workload independently while adapting to changing priorities.
  • Holding a valid UK NMC registration, Primary or Secondary Care experience and a full UK driving licence with access to your own vehicle.

Role Summary

As a Nurse, you will play a key role in delivering expert clinical support to clients and carers, ensuring that care plans are current, effective, and person-centred. You will maintain high clinical standards through hands-on training, continuous assessments, and close collaboration with the wider multidisciplinary team (MDT). This role combines direct clinical care, training, and advisory responsibilities, with participation in the on-call rota.

Main Duties and Responsibilities

Care Planning and Clinical Oversight:

Create and update personalised care plans based on clinical assessments and evolving client needs.

Review and revise medication records in line with prescription changes.

Conduct regular clinical reviews and risk assessments to ensure quality and safety.

Lead initial clinical assessments for new clients, ensuring a smooth onboarding process.

Document incidents and participate in follow-ups and learning discussions.

Review Birdie Concerns and Inbox items to ensure clinical concerns are dealt with or escalated in a timely manner.

Learning and Development:

Deliver on-site and simulation suite-based training to carers and support workers in areas such as medication administration, moving and handling, and other specialist tasks.

Support new carers in achieving the Care Certificate and clinical competencies.

Participate in external and in-house clinical training to advance your professional skills.

Complete clinical signoffs for carers and oversee their development in line with care delivery needs, engaging in Clinical Audits and Quality Improvements.

Client and Carer support:

Provide ongoing clinical guidance and support to carers working in the field.

Maintain strong, supportive relationships with clients and their families.

Offer real-time clinical advice and help troubleshoot any concerns related to client care.

Contribute to maintaining a responsive, person-centred culture of care.

Participate in home reviews and meet and greets.

Support with carer appraisals and provide input into carer welfare and wellbeing

Participate in occasional meet and greets with new or prospective staff.

Liaison and Professional Collaboration:

Work collaboratively with the Head of Clinical Delivery and office team.

Liaise with GPs, district nurses, hospitals and other MDT members to ensure coordinated care.

Participate in multidisciplinary meetings where required to advocate for client needs.

Work alongside and report any concerns to the Designated Safeguarding Lead.

Maintain an active NMC registration, adhere to the NMC code of conduct and participate in training to maintain competency.

On call responsibilities:

Participate in the clinical on-call rota, providing out-of-hours guidance and support.

Assist with crisis management and care escalations when needed.

Experience, skills and knowledge required for the role

Registered Nurse with valid NMC registration, with no restrictions on practice.

Strong clinical knowledge and confidence in working autonomously.

Previous experience in Primary or Secondary Care.

Passion for teaching, supporting others, and person-centred practice.

Excellent communication and problem-solving skills.

Ability to work under pressure, prioritise workloads and meet tight deadlines.

Confidence with digital care systems (for example, Birdie) and MS Office.

Full UK Driving Licence and access to own transport.

Participation in the on-call rota.
