Job summary
A great opportunity for a Registered Nurse looking for more autonomy and the chance to deliver specialist care in the community.
You'll support patients receiving home-based IV therapies, including Total Parenteral Nutrition (TPN), helping them manage their treatment safely at home.

You'll provide clinical care, patient education, and ongoing support to help patients manage their treatment safely at home.
The role includes home visits, monitoring patient progress, maintaining accurate clinical records, and working closely with multidisciplinary teams to deliver high-quality patient care.

You will work with a wider healthcare team, including dietitians, pharmacists, hospital staff, and other professionals, to make sure patients receive safe, high-quality care that meets their needs.
Main duties and responsibilities
- Deliver safe, high-quality nursing care in line with service procedures and clinical guidelines
- Support patients receiving IV therapy and TPN in the community and at home
- Visit patients at home to start treatment, carry out assessments, provide follow-up care, and offer ongoing support
- Teach and support patients, carers, and families so they feel confident managing treatment safely
- Build trusting relationships with patients and provide kind, personalised care
- Keep clear and accurate clinical records, including electronic patient notes
- Monitor how patients are doing and report any concerns or changes straight away
- Use and care for clinical equipment and supplies safely
- Work closely with the wider healthcare team to support the best possible patient outcomes
- Take part in team meetings, service improvements, and ongoing training and development
- Follow safeguarding, infection control, governance, and regulatory requirements at all times
