A fantastic opportunity has arisen for a Band 6 Mental Health Lead Nurse, or an experienced Band 5 looking to take the next step into a Band 6 role, to join the Older Adult In-Reach and Home Treatment Team in Chesterfield.The primary aim of the service is to support older adults experiencing a deterioration in their mental health within their own homes and, whereappropriate, avoid hospital admissions where appropriate.The team also works closely with mental health inpatient wards tofacilitatetimelydischarge and supportpatients inreturning home as soon as it is safe to do so.The serviceoperates7 days a week, from 9:00am to 5:00pm, including weekends and bank holidays.Due to the large geographical area covered by the service and the requirement to respond within 2-4 hours, the successful candidate must be able to travel independently, with access to their own vehicle being essential.

Main duties of the job

The post holder will be clinically responsible for supporting the delivery of a flexible, responsive local service within the community.

You will work collaboratively with colleagues, key partners and those within the local community to develop services to meet the mental health needs of the community.

The post holder will employ a wide range of skills, building on and developing their expertise and actively seeking advice and collaboration with issues which may not be readily within their sphere of competence.

As a senior registered practitioner you are professionally and legally accountable under the NMC code of professional practice.

We cover Chesterfield and North East Derbyshire.

There is an expectation that as a Lead Nurse, you will be able to make complex decisions, lead a shift, cover duty and formulate detailed assessments and person-centred care plans to assist in the prevention of hospital admissions where appropriate.

Experience of working with older adults with a functional, mental health illness is essential.

The successful applicant will work within a supportive MDT and have close links with the Community Mental Health Teams, GPs and social care.

Professional development and further learning is encouraged and you will receive robust clinical and managerial supervision, along with wider support and training in-house within the team.

You will also be expected to provide supervision to team members and mentor students.

The team works 365 days a year, with hours covering, Monday-Sunday, 9.00am-5.00pm; this includes weekends and Bank Holidays when needed.
